NCASA is an affiliate member of the United States Adult Soccer Association (USASA) under the United States Soccer Federation (USSF). One of the benefits of being a player in a sanctioned league is the availability of a supplemental accident insurance policy.
Players should file claims within 90 days from the date of the injury or the benefits may be denied due to untimely filing. Note - you do not have to wait until you have all of your bills to submit the claim form. Bills may be forwarded later with a copy of the original claim form.

To file a claim download a Claim Form (to the right) and follow the instructions to file your claim.
CLAIM FORMS MUST BE PROCESSED THROUGH YOUR LEAGUE AND SENT TO NCASA FOR APPROVAL. Do not send a Claim Forms directly to the insurance company as it will be returned and will delay the process.
Instructions:
- Complete the Claim Form.
- Fill in the League name. The State Association is NCASA. The Region is III.
- Obtain the signature (on page 2) of your Team Captain present at the match at the time of the accident.
- Send the form to your League for signature.
- Your League will validate that you are a current player and will sign the Claim Form.

OR mail the form to NCASA, PO Box 18229, Greensboro, NC 27419. - NCASA will process the form and send it to the insurance provider.
- The player will be contacted by the insurance provider once the claim is processed.
